Certified Copy of Birth Certificate
(for POYFA files)

3 Copies of Final Report Card
(End of 2008-2009 School Year - by July 18th)

Registration Fee
Cheer and Football may pay a non-refundable $50 deposit at registration; balance will be due prior to the first practice.

Total Fees
$200 - Football/$175 if paid in full prior to May 1st

$175 - Cheer/$150 if paid in full prior to May 1st

** FAMILY DISCOUNT-- $25 off each additional child registered
